TVS Sports Physicals for incoming 7th-12th grades for the 2018-19 school year will be May 2nd & 17th at the Preble County Medical Center. 

 

Please stop by the TVS office and sign-up for date and time (6:00 – 8:00pm).  When you arrive, please bring physical form with history section complete.  Also, wear athletic apparel (i.e. shorts, sports bra) and do not consume any caffeinated beverages that day.  Cost for the Phyiscal is $10.00 and will be paid at time of physical (Cash or check only).  Checks should be made payable to DSMI.

 

The money raised from the physicals is donated back to each school.  This is all made possible by the generous donations of our physicians and volunteers.
