Complete Final Forms 2026/27

It is already time to look ahead to the upcoming school year! The online registration through Final Forms is open for the 2026/27 school year. Please take a few minutes to complete and sign your student’s back-to-school forms. Getting this done early ensures your child is cleared for all school activities, field trips, and athletics right from day one.

How Long Will This Take?

Returning Students: About 3–5 minutes. Your data from last year is saved; you just need to review it, make any necessary updates, and sign.
New Students: About 15–20 minutes. You will need to input your basic contact, medical, and emergency info from scratch.

Step-by-Step Directions

1. Locate your Final Forms account: Go to Twin Valley South's Final Forms portal.2. Log in or Register:

Returning Parents: Click Parent Login and use your existing credentials. (Do not create a new account).
New Parents: Click New Account under the Parent icon to register.

3. Update and Sign Forms: Click on your student's name. Locate the forms listed on the left side of the screen. Fill out each section and sign electronically.4. Verify Completion: Ensure all sections turn GREEN. If any section remains RED, a signature or required field is still missing.
